How to interpret a cocaine result in urine and oral fluid
Cocaine is a Schedule II central nervous system stimulant, hydrolysed in the liver to benzoylecgonine, the inactive metabolite most testing programs actually measure. Exposure usually follows insufflation, smoking, or injection, but benzoylecgonine can also arise from sources a program may not expect: coca-leaf tea and other coca-derived products, and topical or ophthalmic cocaine used medically. Contact with contaminated surfaces is a different question, addressed on page 2.
Half-life roughly 1.5 hours. Measured in oral fluid and blood. Deposits on surfaces and currency, so its presence in oral fluid at very low cutoffs is less specific for ingestion.
Benzoylecgonine
Major inactive metabolite and the primary urine target. Requires hepatic hydrolysis, so it indicates cocaine entered the body. This is the analyte that separates ingestion from contact.
Cocaethylene
Formed only when cocaine and ethanol are present together. Detection indicates co-use.
Norcocaine, m-hydroxy-BE
Minor metabolites. Reported on some definitive panels; not used for routine interpretation.
The Interpretive Question
What a benzoylecgonine positive establishes, and what it does not, depends on which analyte and matrix produced it.
The cocaine immunoassay is among the most specific on the panel, so analytical accuracy is rarely where interpretation fails. It fails at the next step, when detection is read as proof of intentional use. Benzoylecgonine is the analyte that carries that distinction, because it requires metabolism rather than contact.
That cocaine entered the body. Benzoylecgonine is a metabolite, so its presence requires systemic absorption and hepatic hydrolysis, not surface contact.
That exposure fell within the detection window. Roughly three days in urine after single use, longer with chronic use.
That the assay performed correctly. Cocaine is among the most specific immunoassays on the panel.1
It Does Not Establish
That exposure was intentional. Topical ophthalmic cocaine produced positive urine BE in 94% of 50 subjects at 4–6 hours.2
That exposure was illicit. Coca-leaf tea yields genuine BE. The result is true; the inference of drug use is wrong.
That no fentanyl was present. A cocaine result carries no information about co-present opioids.3
Anything about dose, timing, or impairment.
Where Interpretation Goes Wrong
Treating urine and oral fluid as interchangeable. Urine measures a metabolite and therefore ingestion. Oral fluid measures parent drug too, and parent drug can reach the mouth without being swallowed.
Reading detection as intentional use. The analyte, the matrix, and the cutoff together determine how far that inference can travel.
Assuming a negative excludes exposure. Immunoassay missed BE above the 300 ng/mL cutoff in 21 of 1,398 specimens on one platform and 29 on another.1
Sanctioning on a screen alone. A presumptive positive is a reason to confirm, not a finding of fact.
Ignoring the fentanyl question. The cocaine result says nothing about the opioid risk.3
Exposure & Testing Considerations
The Contact Exposure Question
Is Currency Actually Contaminated?
Yes, and it has been measured. Across 418 currency samples comprising 4,174 bills collected from 90 US locations between 1993 and 2009, cocaine contamination averaged 2.34 ng per bill.4
Does Handling It Cause A Positive Urine Test?
Not at standard cutoffs. In a deliberate forced-contact experiment, bills were immersed in powdered coca paste, handled repeatedly through a normal day, and no hands were washed. Serial urine reached only 72, 36 and 24 ng/mL benzoylecgonine, far below the 300 ng/mL screening cutoff.5
So Where Does The Margin Disappear?
Oral fluid at low cutoffs. Of 1,819 people, 43.7% were positive for cocaine at a 1 ng/mL threshold, yet among those 794 positives only 29.1% reported use in the past 24 hours. Detection of a cocaine metabolite, rather than the parent concentration, was the strongest predictor of intentional use.6
Does The Environment Leave A Trace?
Measurably, yes. Cocaine was found on 26.8% of 127 mobile phones swabbed at New York nightclubs. Phone detection matched reported use with high specificity (0.85–0.91) but low sensitivity (0.37–0.73), and among people who were oral-fluid positive without reporting use, 17.1% had cocaine on their phone. The authors attribute part of this to environmental sources rather than recent use.7
The cutoff is an interpretive decision, not a technical default. The same specimen can be negative at 300 ng/mL and positive at 50 ng/mL. Lowering a cutoff does not find more truth; it widens the range of exposures that qualify as positive, including exposures the program did not intend to capture.
Highly specific: one false positive in 1,398 specimens, no cross-reactivity observed.1
Coca-leaf tea and coca products yield genuine BE. Analytically true; the inference of illicit use fails.
Topical and ophthalmic medical cocaine. Positive in 94% at 4–6h, 70% at 24h, 2% at 48h, none at 72h.2
False Negatives
Misses above cutoff. 21 of 1,398 specimens on one platform and 29 on another held BE above 300 ng/mL yet screened negative.1
Zinc. Elevated urine zinc, ingested or added, reduces EMIT detection of cocaine, methamphetamine, THC and opiates.8
Timing and dilution. A specimen outside the window, or heavily dilute, may fall below cutoff despite genuine exposure.
Program Guidance
Matrix, Cutoff And Program Guidance
Use urine when the question is whether cocaine was taken. BE requires metabolism, and the 300 ng/mL cutoff sits well above what contact exposure can produce.5
Use oral fluid for recent use, and set the cutoff deliberately.9 At 1 ng/mL the gap between detection and reported use is wide; relying on metabolite detection classifies intentional use more accurately, at some cost in sensitivity.6
Read a positive as exposure, not intent, and confirm before any sanction or termination. Review the medication list and recent procedures first.2
Test for fentanyl separately. The overdose risk is not in the cocaine result.3
State the limits plainly in reports and testimony. An interpretation survives cross-examination when it claims only what the analyte supports.
Background · The Current Picture
Scale of use. An estimated 5 million Americans aged 12 or older reported using cocaine in the past year (2023).10
Mortality. About 30,000 US overdose deaths involved cocaine in 2023, an increase of nearly 7% over 2022 by count.10 As an age-adjusted rate, cocaine-involved deaths rose 4.9%, from 8.2 to 8.6 per 100,000, against 105,007 total overdose deaths that year.11
Why it matters here. Much of that rise reflects opioid co-involvement rather than cocaine alone.3 A cocaine result does not detect fentanyl, so the test that names the drug is not the test that names the risk, which is the fact behind most of the program guidance above.
